Purpose: The Medical Grand Rounds (Multidisciplinary) Lecture Series provides practitioners and health care professionals with an update on topics in rehabilitation medicine by:

Providing continuing medical education activities that assist physicians and health professionals to

  • Enhance their knowledge and skills
  • Improve patient safety and quality of care
  • Meet the requirements of certification and licensure

Transformation of Post Acute Care Services-Alternative Delivery of Cardiopulmonary Rehabilitation Services

Objectives:

At the conclusion of the presentation the attendees will be able to:    

1. Identify current economic determinants impacting the healthcare payment reform in the post-acute settings.

2. Describe various healthcare reform delivery models, specifically, Bundle Payment Care Initiative, a Medicare Demonstration Project, designed to improve care transitions, enhance quality, and reduce readmission.

3.  Recognize the need to re-examine the delivery of cardiac rehabilitation in various post acute settings.

4. Discuss the return on investment regarding alternate delivery of cardiopulmonary rehabilitation services in the age of healthcare transformation.
