Mary Free Bed offers options for rehabilitation at home, based on your needs. 

Our Home and Community Program is an outpatient service for adults and children who will benefit from an at-home rehabilitation program — including physical, occupational, speech and recreational therapies, and social work. Our team of clinicians will work with you to develop personalized goals to focus on, whether you’re at home or out in the community.   

Mary Free Bed at Home provides nursing and therapy services for adult patients unable to leave home.

Mary Free Bed at Home

Home and Community

Mary Free Bed’s Home and Community Program enables adults and children at least four years old to get therapy at home following a brain injury, spinal cord injury or other neurological impairment. The program is also available for musculoskeletal and orthopedic conditions. 

You’ll work with our clinical experts in the comfort of your own home or in another home-like setting — and experience the same outstanding level of care as you would in our medical facilities.  

Home and Community services are offered in the Greater Grand Rapids area: north to Grant, south to Kalamazoo, east to Ionia and west to Grand Haven. We’re available whether your inpatient rehabilitation took place at Mary Free Bed Rehabilitation Hospital or at another facility. 

Our program is accredited by the Commission on Accreditation of Rehabilitation Facilities International (CARF). This rigorous accreditation process is based on specific standards for quality care and patient satisfaction.

How it works 

You’ll work with a team of specialists who understand the physical and emotional challenges you and your family face after a life-changing injury.  

We’ll take the time to understand your individual needs and will develop a therapy plan specific to your functionality and goals. 

Your team will determine which services will round out a successful at-home rehabilitation program. We work with our other outpatient programs to seamlessly transition between home and clinic to meet your needs.  

Your team includes: 

  • Occupational therapists who focus on your fine motor skills and your ability to perform daily activities to help you live independently. 
  • Physical therapists to help you build endurance, improve balance and strengthen muscles. 
  • Recreational therapists to help you get back to doing things you enjoy, introduce you to new leisure activities, and gain confidence in the community. 
  • Speech-language pathologists to help with communication skills, problem-solving and memory issues as well as eating and swallowing. 
  • A care manager who partners with you and your caregivers to help you adjust to your injury or condition.
