Mary Free Bed Guild Fund Mission:
The Mary Free Bed Guild Fund also supports community organizations and activities whose focus is on the education, recreation, health, and well-being of the disabled population in western Michigan.
Mary Free Bed Guild Mission:
Restoring hope and freedom by continuing the traditions of benevolence and compassion through governance of the hospital and support of community and hospital programs.
Purpose:
To provide funding for programs or projects of Mary Free Bed Rehabilitation and the Mary Free Bed Guild.
Grant Amounts:
Grants up to $20,000 will be considered. A limited number of projects/programs may be considered for larger grants.
Application:
The applicant must be a Mary Free Bed employee, a Medical Staff member, or a Guild member.
Projects/programs must be approved by a department or clinical program.
The department or clinical program, including the department manager and vice president, must approve the project/program and all participants.
Hospital operations should not be adversely impacted by the project/program.
Grant Request Considerations:
The proposed project/program:
Will demonstrate a clinical significance in patient care.
Has the potential to be self-perpetuating.
Must be approved by the Hospital Research Institutional Review Board, if a formal research project.
May be considered when other Mary Free Bed funding is not available.
May include other sources of funding (strongly encouraged).
Must contribute to the care, quality of life and improvement of services for physically affected persons.
Processing of Requests:
- Review grant request with department leadership for approval.
- Submit Grant Request application via online portal: https://www.maryfreebed.com/internal-grant-application-form/
- Guild Fund Board will review requests during their October and January meetings.
- Notice of the status of your grant request will be received within thirty (30) days of the Fund Board meeting in October and January.
Evaluation and Reporting of Approved Grants:
Provide a progress report within six months of project/program funding approval. These should be sent to the Guild Administrator at Guild.Administrator@maryfreebed.com.
Include the following:
- Grant title or topic
- Date awarded
- Did the project meet your goals and expected outcomes? How and why.
- What metrics or data has been gathered as it relates to the goals and outcomes?
- Were you able to implement the grant as planned?
- Was the level of funding adequate?
- How do you see the project evolving from here?
- Proposed budget to actual for the program or project.
- Upload/attach a photo(s) of your grant in action.
