Patient Rights + Responsibilities

Mary Free Bed restores hope and freedom through rehabilitation. We are partners in your care. We will work together to share information, answer your questions, and help you make decisions about your care. You have the right to:

Access to Care

  • Access reasonable care and treatment in a respectful manner. 
  • Receive quality health care services without discrimination. 
  • Other care options when Mary Free Bed services no longer meet your needs. Mary Free Bed will not transfer you as an inpatient until another site agrees to accept you, and we will talk to you about any risks and benefits. 
  • Take part in research studies with complete information. You will continue to receive quality care if you choose not to participate. 
  • Agree to, or refuse, a treatment as allowed by law. 
  • Ask about charges and payment options
  • Know who to ask for questions or problems about your care.

Communication

  • Information that you can understand. 
    An interpreter. 
  • Have visitors for social and emotional support. 
  • Ask Mary Free Bed to call your physician/family to tell them you are a patient of Mary Free Bed. 
  • Know the names and roles of each person on your care team.
  • A clear explanation of your condition. 
  • Plan and take part in your care, and to understand your treatment plan.  
  • Your family or patient representative being involved in your care, if you can’t make decisions for yourself. 
  • Understand outcomes and any risks of your treatment. 
  • Include or exclude family members in your care decisions.

Confidentiality and Medical Records

  • Expect your medical records will be kept private.  Only minimum necessary information will be shared with others. 
  • You may ask for copies of your medical records and receive them in a reasonable amount of time. 

Pain Management

  • The most appropriate way to assess and manage your pain.

Privacy, Safety and Respect

  • Personal privacy and receiving care in a safe and secure setting. 
  • Be free of any type of restraint or seclusion unless your doctor says it is needed to keep you safe.
  • Be free of any form of neglect or abuse. 
  • Practice your personal and cultural beliefs and preferences. 
  • Access religious or other spiritual services.

Advance Medical Directives

  • An advance order for decisions about your health, such as a living will or durable power of attorney.

Complaints-Grievances

  • File a complaint with the Director of Quality & Risk. We will investigate your complaint and respond within 7 days.
  • A patient or representative may file a complaint with a state agency.

Your Responsibilities

  • Tell us everything you know about your health and condition.
  • Ask questions if you don’t understand what you are being told. 
  • Tell us if you have an advance directive, such as a living will. 
  • Follow your doctor’s orders, advice, and prescribed treatments.
  • Tell your care team if you notice a change in your condition. 
  • Work with your care team to manage your pain. 
  • Be respectful and civil in language and conduct.
  • Follow instructions, policies, rules, and regulations.
  • Pay your bill promptly, or talk with the billing staff if you have questions or need a payment plan.
