Inpatient Admission Criteria

General admissions criteria for Mary Free Bed Rehabilitation Hospital’s inpatient programs (not including Sub-Acute Rehabilitation) are:

  • Patient is medically stable at time of transfer
  • Patient demonstrates rehabilitation potential
  • Patient is capable of participation in a minimum of 3 hours of therapy per day
  • Cognitive level: Adult Brain Injury – Cognitive levels between III-VIII on Rancho Los Amigos Scale of Cognitive Functioning
  • Pediatric/Adolescent Brain Injury – Cognitive levels between II-VII
  • Patient demonstrates potential for home-like discharge
  • Identification of insurance for prior authorization of transfer to Mary Free Bed Rehabilitation Hospital

    Please note that patients frequently fall into gray areas and admission is determined on an individual basis. Special criteria apply for inpatient admission to our Sub-Acute Rehabilitation Program. Please call to speak with an admission liaison to discuss questionable cases.

    The admission liaison will continue to call nursing units and therapies to further clarify a patient’s status, rehabilitation potential and gather information for insurance prior authorization. Local referrals are generally seen by a physiatrist to determine medical appropriateness for admission.

Admission Criteria for Ventilator Patients

Medical Stability and requirements:

  • Fio2 40% or less and stable for 48 hours
  • Peep 10cmh2o or less and stable for 48 hours.
  • PH 7.35 – 7.60
  • No other medical complications (fever, planned procedures, critical labs, etc.
  • No recent increase in ventilator settings (except for weaning purposes).
  • Pediatric Vent Admissions coming into MFB from home.
    • Approval from MFB PMR physician for admission.
    • Review case with Respiratory Pediatric Medical director for approval!
  • Pediatric ventilator referral from acute care (not from HDVCH).
    • Approval from MFB PMR physician for admission.
    • Review case with Respiratory Pediatric Medical director.
    • Respiratory Pediatric Medical director to coordinate transfer to HDVCH PICU to transfer patient to home ventilator and assess for stability and then will coordinate to MFB via above process.

 

Admission process and checklist

  • (2) dedicated caregivers/family members are identified to care for the patient 24/7 after discharge from MFB
  • In-person or virtual visit with patient/caregivers with Liaison and RT
    • Discuss expectations and training once at MFB.
    • Tour of Mary Free Bed pediatric unit for family
  • Baseline CXR and Blood gas (48) hours before admission.
  • No pediatric ventilator admissions on holidays, Friday, Saturday, or Sunday.

 

Admission criteria for Adult Ventilator Patients:

Medical Stability and requirements

  • Fio2 40% or less and stable for 48 hours
  • Peep 10cmh2o or less and stable for 48 hours.
  • PH 7.35 – 7.60
  • No other medical complications (fever, planned procedures, critical labs, etc.)

 

Admission process and checklist

  • In-person or virtual visit with patient/caregivers with Liaison and RT to set up expectations and training once at MFB.
  • Baseline CXR and Blood gas 48 hours before admission.
  • Pulmonary Sign Off
  • No ventilator admissions on holidays, Friday, Saturday, or Sunday for vented patients.
    • *Fridays can be approved on a case-by-case basis if pulmonary and attending PMR physicians both approve.
